Older gentlemen with HX of two previous spine surgeries.
First, a Laminectomy followed by a second, repeat laminectomy with posterior lateral fusion L3-5.
Two years pass and the patient presents with worsening back pain, was found to have pseudoarthrosis at L3-5 with bilateral L5 screw loosening.
I had the following goals for his surgery:
- Place a large interbody implant for optimal mechanical stability
- Revise the posterior instrumentation and upsize screws
- Avoid the spinal canal and previous scar tissue
- Minimize anesthesia time, blood loss and overall morbidity
In the prone position, I performed both the fusion using the lateral interbody technique and revised the pedicle screws. In this specific case we decreased the operative time by 1.5 hours, while achieving optimal stability.
