Three patients, each with an L4-5 issue that benefited from surgical management.

- Young patient with her 3rd reherniation at L4-5 requiring direct posterior decompression. Stabilization was achieved via the TLIF approach. Lordosis was restored via wide release and intradiscal distraction. With young, strong bones, implant subsidence is less of an issue.
- An elderly female with right sided asymmetrical disc space collapses at L4-5 resulting in foraminal stenosis and lumbar radiculopathy. Patient without instability. A standalone ALIF was used to restore foraminal height while minimizing surgical time and morbidity.
- A male in his 60s with a L4-5 mobile spondylolisthesis and dynamic radiculopathy. Disc space distraction was achieved via the prone lateral interbody approach. Posterior instrumentation was used to apply the final reduction maneuver to restore segmental alignment.
